Let's face it-- IEP Meetings are a HUGE part of being a Special Education Teacher. Once school begins, we are in paper work over load! During the school year I made notes of things that would help me be more organized & have data to support everything I do. I decided to take those ideas & create a Back To School Kit, for Special Education.
I included a general form to send home with parents so I can get updated information regarding who to call & how to contact parents for upcoming IEP meetings. I made the form so it wasn't the same ole' boring forms.
Next, an IEP information sheet was included. This form is probably my favorite because it lists the due dates for Annual Reviews & Re-evaluations. This form even gives "extra information" to explain what that those meetings are for. I really wanted my parents to have more information about their child & to know what is coming. I've also had several families to transfer in/out & they were not aware that their child's IEP was due during that transition. I would like to give them "cheat sheets" so they can be more prepared regarding their child's IEP.
Third, a meeting preparation sheet is included. This document is pretty awesome! It gives the proof of documentation of those phone calls, letters, emails, etc. that you have made to schedule your meetings. Now yes, I have a parent contact book where I list my messages BUT this is specifically targeting your meeting & you can bring this form in for your meetings. As much as I stay positive with parents, you need to be prepared for those parents that may claim they have "not heard anything about a meeting". This is great documentation! It also has an area where you can jot down notes for the meeting.
